forms must be turned in to the Athletic Office before
you can participate in any sport. You must also
go to the Athletic office and pick up the OHSAA Athletic
Eligibility Bulletin. You are required to read
this bulletin. This form does not need to be returned.
Your signatures on the Athletic Participation Form acknowledges
you read it.
OHSAA Pre Participation
Physical Exam Form...
A
doctor's physical examination is necessary to verify
that your athlete is physically able to participate
in sports. If you are aware of special medical concerns
of your athlete, we ask that you make the coach and
athletic trainer aware of the problem. This is very
important for us in handling treatment of an injury
or medical problem. (two-sided form). Click
